I have enjoyed serving as your Physician. Due to various circumstances, I have determined that it is in the best interest of my career to part ways at this time from Modern Vascular. As of July 31st, 2022  I will no longer be a member of Modern Vascular. This does not necessarily mean that you will lose me as your Physician, as I will still be practicing in the Houston area. Once I have a practice address, I will provide this to you by email and regular mail.

 

You may also continue your health care through Modern Vascular and that group will see to it that you are transferred to another Physician. If you choose to be seen elsewhere, you may request to have your records transferred to your preferred Physician. Please let Modern Vascular know at your earliest convenience, which of these options is best for you.
